DELAWARE-MUNCIE METROPOLITAN PLAN COMMISSION

PRIMARY APPROVAL - PRELIMINARY PLAT

INSTRUCTIONS 

The application will be submitted to the Commission when the following items have been filed with the Plan Commission Office:

**If you are filling out the forms on-line, they still must be PRINTED OUT and brought to the Plan Commission Office.  To print the form, right-click on the form and select PRINT.  When filling out the forms online, Please disregard all sections headed with BRIGHT RED TEXT.

1. One (1) copy of the application for Primary Approval.

2. One (1) copy of any proposed Plat Restrictions/Covenants.

3. One (1) digital copy of the plat in AutoCAD 2000 or newer, dwg or dxf file formats.  Digital plat must be registered or drawn based upon a known coordinate and stored in the Indiana State Plane (East) Coordinate System.

4. One (1) copy of approved drainage plan.

5. Thirty-five (35) copies of the preliminary plat on a scale of at least 1' = 100" showing the following information:

A. Location of the plat by section, township, range, etc.; the North point, scale and date; and the proposed name of the subdivision - unduplicated by others - and names of subdivider, the owner and the registered land surveyor or licensed engineer, with registration/license number.

B. Boundary lines with bearings, distances and acreage; the location, width, names of existing streets, RR & utility rights-of-way, buildings/structures, and section or municipal lines adjacent to the subdivision; and names of adjacent subdivisions with plat book record data.

C. Existing contours of not more than 5' where the slope is greater than 10% and 2' where it is less than 10%, using the U.S. Coast and Geodetic Survey datum.

D. Block, outlots, and lots consecutively numbered or lettered; building setback lines, utility and drainage easements, public use areas (parks, etc.), and zoning; all new streets with unduplicated names, rights-of-way, roadway widths, approximate grade and gradients.

E. Utilities including sanitary and storm sewers, regulated drains, other drainage facilities, water lines, gas lines, and electric lines - existing and proposed - where the size and capacity of each shall be shown and distances indicated.

6. When public sanitary sewers are not available, a report from the Delaware County Health Department concerning use of septic systems; and when the drainage system contains off-site facilities, a written grant of easement from the affected surrounding property owners.

7. Names and addresses of adjoining property owners on stamped envelopes. One copy of a completed Notice of Public Hearing form which will be returned to the subdivider for publication.

8. The filing fee is paid under established fee schedules.

ADDITIONAL INSTRUCTIONS FOR PLAT DRAWING

FLOOD AREAS: When the site contains a flood plain according to the Federal Emergency Management Agency (FEMA) maps adopted by reference with the local flood management ordinances, the Floodway and the Floodway Fringe, as shown on said FEMA maps shall be delineated on the plat. The applicable regulated drain easement must also be shown. The 100 year flood plain line should also be indicated, which is the same elevation line delineating the floodway fringe.

restricted area. Prior to any work in this
area, Construction in a Floodway permit
approval must be obtained from the State
Dept. of Natural Resources.]


RESTRICTIONS & OWNER'S CERTIFICATE: When a plat contains a flood hazard area according to the FEMA maps, the following statements are required:

OWNER'S STATEMENT - A floodway is hereby established as shown on this plat which shall not be disturbed in any manner unless authorized by the Indiana Department of Natural Resources. A floodway fringe is hereby established as shown on this plat within which area, the lowest floor elevation shall be set at or above ______ feet, being at least 2 feet above the regulatory flood elevation. Flood insurance may be required within the floodway fringe, which can be excessive.

RESTRICTIONS - No construction, excavation, fill or similar activity shall occur within the designated floodway unless properly authorized by the State of Indiana. There shall be no alteration of any drainage facility within this subdivision unless authorized by the appropriate governmental agency.

WHEN IMPROVEMENTS HAVE BEEN COMPLETED:

COUNTY COMMISSIONERS/BOARD OF WORKS:   THIS IS TO CERTIFY that the Board of Commissioners of Delaware County, Indiana/Board of Public Works and Safety of the City of Muncie, Indiana hereby approves and accepts the dedication of public lands and public improvements within the foregoing plat of  ________________________________________________________________________.

NOTE: Where there are no public improvements under the jurisdiction of the above Boards, delete the phrase "and public improvements" from the above statements. ALSO, during the final plat secondary approval process, the Board of Commissioners or the Board of Works signatures should be obtained prior to Plan Commission action to provide verification to the Commission that the public lands/improvements have been accepted. If the plat is not signed, a Certificate of Acceptance of Public Improvements must be signed prior to Plan Commission action.
