To receive immunizations, a record of the dates of all previous immunizations MUST accompany the child. if any adult other than the parent or guardian brings the child for shots, they must have WRITTEN PERMISSION FROM THE PARENT OR GUARDIAN. The note should include the names of the perent, the child and the adult accompanying them, as well as the date of the clinic.
