To receive immunizations, a record of the dates of all previuos immunizations MUST accompany the child. If any adult other then the parent or guardian brings the child for shots , they must have WRITTEN PERMISSION FROM THE PARENT OR GUARDIAN. The note should include the names of the parent, child and the adult accompanying the child as well a the clinic date.
