To receive immunizations, a record of the dates of all previous immunizations MUST accmpany the child. If any adult other than the parent or guardian brings the child for shots, they MUST HAVE WRITTEN PERMISSION FROM THE PARENT OR GUARDIAN. The note should include the names of the parent, child and adult accompanying the child as well as the clinic date.
