To receive immunizations, a record of the dates of all previous immunizations MUST accompany the child. If any adult other than the parent or guardian brings the child for shots, they MUST HAVE A WRITTEN PERMISSION NOTE FROM THE PARENT OR GUARDIAN. The note should include the names of the parent, child and adult accompanying the child as well as the clinic date.
