NOTE: Those individuals who plan to have the glucose and/or cholesterol test should fast at least 8 hours, or do not eat breakfast, but may drink water. For those parents planning to get their child immunized, a shot record of all previous immunizations must be presented. If someone other than the parent or guardian brings the child for their shots, they must have written permission that includes the name of the child, the parents name, the date and the person responsible for the child at the health fair.

The health fair is specifically for those who are uninsured, homeless, in transitional housing or experiencing financial difficulties . Please attend this health fair so we can help you during this difficult time.
