To receive immunization, a record of the dates of all previous immunizations MUST accompany the child. If any adult other than the parent or guardian brings the child for shots, they must have WRITTEN PERMISSION FROM THE PARENT OR GUARDIAN. The note should include the names of the parent, child, and adult accompanying the child as well as the clinic date.
Kindergarten booster shots are given at age 4-6 years. The 2nd MMR and HEP-B series are given to children through 18 years of age. This requires a complete shot record with proper adult permission.
