WHO CAN OBTAIN A BIRTH/DEATH RECORD
 
In accordance with I.C. 16-37-1-8, certified copies of a birth/death record can be issues to an individual IF:

 

1)       The individual can complete the appropriate request form with proper information to identify the record;

2)       The applicant can provide at least one form of PHOTO identification;

3)       The applicant is:

a)       The person names on the record (over the age of 18).  An individual under the age of 18 may obtain their own record if

They have proof of emancipation.

b)       Parents of the person named on the record (parents name must appear on the record)

c)       Sibling (over 18) with proof of relationship to at least one parent

d)       Grandparent (must be the parent of a parent of the person named on the record) with proof of relationship

e)       Child (over 18) of person names on record, with proof of relationship

f)         Spouse, with proof of relationship (example: current insurance card, vehicle registration, bank/checking card,

birth record of a child marriage license, etc)

g)       Court appointed legal guardian (must provide guardianship papers with seal)

h)       Attorney representing person named on record (must of ID for self and ID/permission note from person names on record)

i)         Caseworker from Division of Family & Children with court appointed guardianship papers (must show ID)

j)         Law enforcement personnel with ID and court order

k)       Other county agencies - left up to discretion of Local Health Department

 

How to verify relationships:

 

  1. Grandparent -

a. If the parent names on the record (the child of the requestor) was born in Delaware County we will look up his or her record to verify

                the requestor is listed as a parent on the birth certificate.

i.                     EX:  Agnes Smith comes to our office to request a birth certificate for her granddaughter Sally Jones.  Agnes says she is the maternal grandmother (Sally's mother's mother).  When we search our record, we find Sally's mother's maiden name is Judy Miller,  We tell Agnes her name is different than Sally's mother, she explains her first husband Mr. Miller died and she married Mr. Smith.  Since Agnes' daughter (Sally's mother) was born in Delaware County we are able to research her record and verify Agnes is the mother of Judy Miller, which makes her the grandmother of Sally Jones.

            b. If the requestor was born outside Delaware County but in Indiana, we may call the state or local health department to verify information, if time permits or ask the requestor to

                provide a copy of his or her child's birth certificate to verify he or she is indeed the parent. 

            c. If the requestor was born outside of Indiana, his or her child's birth certificate is needed to verify that he or she is indeed the parent.

 

  1. Siblings -

a.       If the requestor was born in Delaware County we will look up his or her record to verify that they do indeed have at least one parent in common.

b.       If the requestor was born outside of Delaware County but in Indiana, we may call the state or local health department to verify information, if time permits or ask the requestor

to provide a copy of his or her birth certificate.

c.       If the requestor was born outside of Indiana, he or she will need to provide a copy of his or her birth certificate showing they have at least one parent in common.                                                    

     

  1. Spouse -

a.       If the couple has any children born in Delaware County, we will look up that child's birth certificate to verify the couple was married on the birth record.

b.       They may show a birth record from outside of Delaware County of a child as well.  Remember on a certificate of a child born in wedlock, both parents will most likely have the same name.  If this is not the case, then this will not be an acceptable verification.

c.       A copy of the marriage license.

      

  1. Adult Child of -

a.       If the requestor was born in Delaware County we will look up his or her record to verify the parent named on the record.

b.       If the requestor was born outside Delaware County but in Indiana, we may call the state or local health department to verify information, if time permits or ask the requestor to provide a copy of his or her birth certificate.

c.       If the requestor was born outside of Indiana, he or she will need to provide a copy of his or her birth certificate showing that the record is listed as the parent.
